ADDITIONAL INSTRUCTIONS FOR NONIMMIGRANT V VISA APPLICANTS

This Embassy has received notification from the Department of State of your eligibility to apply for a V visa. You are required to prepare for the visa interview and medical examination by taking the following steps:
Collect the following documents and place a check mark by each collected document to show that you have it in your possession. PLEASE DO NOT SEND ANY DOCUMENTS WITH THIS FORM. YOU ARE REQUIRED TO BRING THE DOCUMENTS TO YOUR VISA INTERVIEW

Original receipt notice, form I-797, confirming the approval of a family based second preference petition in your name.

Passport,
valid for at least six months and endorsed by the issuing authority
for travel to the United States. British regular passports meet this
consideration.

Birth Certificate*
showing your date and place of birth and your parents' names. Please
note that the so-called British "short form" birth certificate is
not acceptable. See also IV15 Supplement 1 for additional information
about availability of birth certificates.

Adoption Certificate*
if applicable, showing date and place of adoption, along with the
names of your adoptive parents. This certificate must be issued by
a public authority and show that a public record exists of your adoption.
- Deed Poll,
or other legal evidence if applicable, showing your change of name
(except by marriage).

Marriage Certificate*,
for each of your prior marriages.

Divorce Decree or Death Certificate*,
which prove that all prior marriages have ended.

Police Certificate,
A separate certificate is usually required
from the police of each country in which you
have lived for twelve months or more after the age of 16 (the period
is six months if country of birth
or current residence). This certificate must state whether you have
a police record in that country.

Court Record,
If you have been convicted of any crime in
any country, a copy of the court record of each conviction is required,
even if you have subsequently benefited from an amnesty, pardon or
other act of clemency such as the Rehabilitation of Offenders Act.

Military Record,
One certified copy of any military record
should be obtained covering periods spent in military service, including
wartime or national service.

Three
Color Photographs. These photographs
must have a white background and must be glossy, unretouched, unmounted,
and identical. The dimension of facial image should be about 1 inch
from chin to the top of the hair; the subject should be shown in 3/4
frontal view showing the right side of face with the right ear clearly
visible. No earrings should be worn.

Evidence of Support.
Please read and follow the directions given in the enclosed form DS-2001 entitled "Evidence which may be Presented to Meet the Public
Charge Provisions of the Law." Note that U.S. sponsors must complete
I-134 for those applicants who are providing affidavits of support
as evidence that they will not become a public charge. Affidavits
of support and evidence of prearranged employment must be less than
one year old when presented to the consular officer.
- Translations.
All documents not in English must be accompanied by certified translations
into English. Translations must be in single copy and certified by
a qualified independent translator and sworn to before a Notary Public
or Commissioner of Oaths.
All civil documents
furnished must be issued by a public authority. Those originating from
the United States must have been obtained from a State Vital Statistics
Office or a city, county, or other local office. Certified copies of
British documents are obtainable from the General Register Office, Smedley
Hydro, Trafalgar Road, Southport, Merseyside, PR8 2HH, or from the local
registrar's office in the district of registration.
CHILDREN:
Each child who is deriving status from the immigrant visa petition filed
on your behalf and who will apply with you for a V visa requires the
following:
Passport (See
item 1 on page 1. Children under the age of 16 can be in the passport
of the immigrating parent.)
Birth Certificate
(See item 2 on page 1.)
Adoption Certificate
(See item 3 on page 1.)
Deed Poll
(See item 4 on page 1.)
Police Certificate
(See item 7 on page 1. Children over the age of 16 require the
same type of police certificates as their parents.)
Court Record
(If a child has ever been convicted of any crime, see item 8 on page
1)
Military Record. (See item 9 on
page 1.)
Three Color Photographs.
(See item 10 on page 1.)
Evidence of Support.
(See item 12 above.)
Translations.
(See item 13 above.)
Optional Form 156,
"Nonimmigrant Visa Application". (Complete one copy for each child,
but do not sign the forms.) You will receive the DS-156
with the appointment letter.
Custody Decree.
If you are applying for a visa for your children by a previous marriage
and if a court has previously restricted your custody of these children,
you should present proof that you have been awarded custody by the court
for the children and that they may be taken out of the United Kingdom.
You are required
to bring to the visa interview your passport and photographs as specified. You are also required
to bring with you the original of all other documents, plus one copy. The originals will be
returned to you if you furnish a copy of the document.
IMPORTANT NOTE:
Visa issuance is not guaranteed.
No assurance can be given
in advance that a visa will be issued to you. Only after all your documents
have been checked, your medical examination completed, and you have
been interviewed by a U.S. consular officer, can a decision be made
regarding your eligibility to receive a visa. You should NOT
give up your job, sell your property, or make final travel arrangements
until you get your visa. In general, the V visa will be a multiple entry
visa valid for travel for ten years from the date on which it is issued.
In the case of a child, the visa will expire on the child’s twenty-first
birthday.

EACH ITEM WITH A CHECK MARK, IMMEDIATELY COMPLETE THE SECTION BELOW
AND SIGN, THEN RETURN ALL PAGES (1, 2, 3) OF THIS FORM TO THE
EMBASSY. PLEASE DO NOT SEND ANY DOCUMENTS WITH THIS FORM. YOU ARE REQUIRED
TO BRING THE DOCUMENTS TO YOUR VISA INTERVIEW.
IF YOU CHANGE YOUR
ADDRESS ANY TIME AFTER THE SUBMISSION OF THIS FORM, PLEASE INFORM THIS OFFICE PROMPTLY.
PLEASE QUOTE YOUR FULL NAME AND DATE AND PLACE OF BIRTH WHEN CORRESPONDING
WITH THIS OFFICE.
