Additional Requirements for Mail Applicants

Children Under the Age of 14

The following additional documents are required if submitting a child's application by mail:

  • Child's long form birth certificate;
  • Photocopy of the biographic page of parents' passport and UK visa or entry stamp page, if applicable;
  • A photocopy of the parents' U.S. visas if applicable;
  • Employment letters, bank statements, property title deeds etc of the parent and a brief cover letter identifying the documents; and
  • If the child is adopted, a copy of the final court order and adoption certificate;
  • If one parent is an American Citizen or the child was born in the United States, the parent must enclose proof that a U.S. Embassy or Consulate has already determined that the child is not an American Citizen.

Please note that if anyone other than the child's parent is submitting the application, that person is required to schedule an appointment to appear in person on behalf of the child.

Adults aged 80 and over

The following additional documents are required if submitting an application by mail:

  • Bank statements, property title deeds, mortgage statements and any other evidence of intent to depart the U.S. after a temporary stay;
  • A letter clearly stating the purpose and duration of travel. If planning an extended visit (more than 3 months) please provide an explanation; and
  • State whether you own property or a business in the United States and/or whether you intend to purchase a business in the United States.
