Is your passport lost or stolen?
If your passport contained a valid U.S. visa you will need to report the loss or theft to the Embassy. To assist us we would like you to complete a report of the loss or theft which you may either email it or mail it to the Embassy or

bring it with you on the day of your visa interview. To find out more, please visit our website.
It is important to note that if your passport is reported lost or stolen and then recovered, it can no longer be used for travel to the United States. If you are traveling visa free under the Visa Waiver Program and you attempt to enter the passport details in the Electronic System for Travel Authorization, your registration will automatically be rejected. You are required to obtain a new passport and visa, if required.
If your U.S. passport is lost or stolen, you should report the loss or theft to the American Citizen Passport and Citizenship Unit

I was issued with a 2 year non immigrant visa but lost my passport. I wish to have the visa re-issued so I can travel. Can anybody help me know what procedure I need to go through please??
Akpo – If you have lost your passport, you should report the loss to your local police and to the Embassy, using this form. The Embassy does not reissue visas. If you wish to continue to travel to the United States, you will be required to obtain a new passport and visa. Once you have obtained your new passport, you should contact the Embassy’s operator service on 09042 450 100 (£1.20/min) to arrange an appointment to apply for a new visa.
My passport went to the washing machine, i gonna make a new cause i think the electronical composant maybe damage!
I have a j-1 visa, which didn’t have many damages, all is clear and visible, only one letter of my name might not be visible!
I come back for christmas, and i’ll not have time for ask a new j-1 visa!
Can i travel with both passport, one with the effective visa and the new one without visa!????
Julien – if your visa has been damaged you will be required to make a new application for a visa before attempting to return to the United States. Please note that any damage to visa visa, including water marks, will invalidate the visa.
So if i still wanna come back for christmas, do i have to make all the process again?Do i have to pay the sevis fee again? Is there an other special process when re-askig for J-1 visa? thanks
Julien – as previously stated, if your visa has been damaged you will be required to make a new application for a visa before attempting to return to the United States. Please note that any damage to the visa, including water marks, will invalidate the visa.
Full details of the application procedure are available from the Embassy website, at: http://london.usembassy.gov/nonimmigrant-visas.html
Hi,
I am about to re-apply for a US Visa. I have a criminal record, and was previously issued with a 10 year B-2 Visa in 2010. Unfortunately, that Visa has now been cancelled as I lost my UK passport. My question is, will I need to obtain another ACPO Police Certificate detailing my record? My circumstances have not changed since 2007 and thus remain the same as last year’s successful application.
Christian – as you will be making a new application, you will be required to present a new ACPO certificate. When you contact the Operator (09042 450 100, £1.20/min) to arrange an appointment, you will be advised further on the documents required when applying for a visa.
